Study on Synovial Fluid Metal Ion Levels and Diagnose Aseptic Loosening in TKA Wins AAHKS Annual Meeting Surgical Award

November 08, 2024 (Dallas, TX) – A study presented at the 2024 American Association of Hip and Knee Surgeons (AAHKS) Annual Meeting seeks to examine if synovial fluid metal ion levels could be used to diagnose aseptic loosening in cemented TKA. Ryan Martin, MD, and co-authors Aleksander P. Mika, MD, Courtney E. Baker, MD, Stephen M. Engstrom, MD, Jaquelyn S. Pennings, MD, Gregory G. Polkowski II, MD, MSc and Jacob M. Wilson, MD received the 2024 AAHKS Surgical Techniques and Technologies Award at the 2024 AAHKS Annual Meeting for their paper, “Synovial Fluid Metal Ion Levels and Diagnose Aseptic Loosening in TKA.”

“My co-authors and I are honored to receive the AAHKS Surgical Techniques and Technologies award for our manuscript on synovial fluid metal Ion levels as a marker of aseptic Loosening in TKA. In this prospective trial, we found synovial levels of Cobalt and Zirconium were elevated in patients with aseptic loosening compared to patients with well-fixed implants,” says Aleksander Mika, MD.

Forty-three patients were enrolled in the study who were undergoing revision of a cemented primary TKA. Synovial fluid was obtained at the time of revision surgery and analyzed for metal ion concentrations. Twenty patients had intraoperatively confirmed aseptic loosening.

In conclusion, synovial fluid Cobalt and Zirconium levels appear to be valuable indicators of aseptic loosening. In the absence of available confirmatory tests, synovial fluid analysis appears to offer a promising diagnostic modality. To further improve the reliability and accuracy of this diagnostic approach, the study will be expanded to include a larger patient cohort. Ultimately, this could lead to better patient outcomes and more effective detection of aseptic loosening following primary cemented TKA.
